  • Orientation week will occur the week of September 1-4, 2020. More information will be sent via email regarding additional requirements to be completed prior to orientation week.

Please Note:

  • The expectation is that all forms are submitted complete by the stated deadline(s)
  • Failure to submit required documentation by the identified deadline(s) may result in withdrawal from the program or the application of a $50 late fee.

TUITION DEPOSIT

BACKGROUND
Upon accepting your offer of admission to the program, learners are required to pay a $500 Tuition Deposit to secure your place in the program. This deposit is non-refundable and the total amount will be credited towards your first semester’s tuition.

ACTION ITEMS
Deposit payments to be completed in Mosaic.
Once you have logged into MOSAIC go to 'Student Centre' and then the 'Accounts' page.
It can take some time after you accept your offer for the tuition deposit fee to show in your account. You can still pay the deposit, just note that your account will show a $500 credit (which appears as -$500). Once the deposit fee is applied your account will balance out to $0.00.

FINAL TRANSCRIPT

BACKGROUND
In addition to a copy of your transcript being submitted to ORPAS, McMaster University requires an official transcript mailed directly from your post-secondary institution to confirm that you have cleared academic conditions (e.g. Degree Conferred by June 30).

Please note:  If your degree has been previously conferred on or before January 2020, you are not required to re-submit an official transcript to McMaster University.

ACTION ITEMS

  1. Request a copy of your transcript from your post-secondary institution
  2. Have your post secondary institution mail a copy of your transcript to the admissions coordinator (as per submission instructions)
  3. Transcript must be received in a sealed envelope directly from your post secondary institution.

VULNERABLE SECTOR CHECK

BACKGROUND
McMaster University requires all Faculty of Health Science students to submit a Police Records Vulnerable Sector Check (VSC), in which the date of search is after the students offer of admission to the OT program and must be submitted by July 31, 2020. VSCs can take up to 16 weeks so please apply early.

Students are required to carry a valid VSC throughout the 2 year program regardless if they are on placement or not.

Certain clinical placements may require a VSC to be updated more frequently than McMaster University standards. Students will need to comply with the clinical site requests at their own expense.

COVID -19 MODIFIED POLICY

All incoming students are to provide an original copy of their Police Records Vulnerable Sector Check (VSC) to the FHS Professionalism Office by July 31st. The VSC must be dated after May 1st and the requesting institution must be McMaster University (eg. cannot be a VSC issued for a summer job). Also note:

  • You must take a copy of the McMaster Request for a VSS letter to your Police Services when requesting a VSC;  if applying to Toronto Police Services or if the Police Service requires a personalized letter please contact fhsprof@mcmaster.ca.
  • You may need your VSC for placement sites so if unable to obtain 2 original copies of your VSC please submit your one original with a self-addressed stamped envelope to the Professionalism Office and your original will be returned to you.
  • Police checks are valid for one calendar year from the date of issue, need to be valid for the entire duration of each placement, and must be updated on an annual basis prior to the current VSC expiring.
  • If unable to obtain a VSC because your Police Services is not currently offering this service please contact fhsprof@mcmaster.ca

HEALTH SCREEN RECORD

BACKGROUND
All learners in the MSc Occupational Therapy Program are required to submit a Health Screening Record.

COVID -19 MODIFIED POLICY

Incoming Health Professional students are required to complete the Year One Health Screening Record and to submit this form by July 15th at the latest. Although it may be difficult or not possible to visit your family doctor right now due to COVID-19 restrictions, please gather as much of the required information as outlined on the form, including lab results, TB testing, vaccination records, previously completed immunization forms, etc. that you have available. We also suggest you contact Public Health (if you grew up in Canada) for any records they may have on file. If you are able to visit your family doctor (in-person or virtually) to have the form completed before July 15th please do so. If not, please complete the student section of your form, then attach all the documentation you were able to locate and submit by July 15th even if not 100% complete.

ANATOMY CARD/HOSPITAL ID BADGE

BACKGROUND
Each student requires an anatomy card/hospital ID badge for numerous purposes throughout the program. This card is personalized to each student. More information will be sent to students about how these cards will be produced and provided.

CLINICAL EDUCATION PLACEMENT FEE

BACKGROUND
All learners in the MSc Occupational Therapy Program are required to pay a one-time, non-refundable clinical education placement fee.

ACTION ITEMS

  1. A payment of $610.00 ($600 for Clinical Education Fee and $10 for Anatomy Card/Hospital ID Badge) is due to the program. This fee is non-refundable. The Program is currently exploring options for the payment of this fee given restrictions due to COVID-19. Once payment option(s) are finalized students will be notified of next steps.
  2. Students will receive their anatomy card/hospital ID badge during one of the in-person classes, hopefully during the fall term. Photos for the anatomy card/hospital ID will need to be provided to the Program Coordinator. Photos must meet the following criteria:
    • Medium to high resolution
    • Head-shot only (i.e. from the shoulders up)
    • If you wear glasses please ensure there is no glare
    • The background of the image should have a high contrast to the image of the student
    • In the event that the system rejects the photo, you may be requested to provide an alternative photo

CPR CERTIFICATION - LEVEL C OR BLS

BACKGROUND
The OT program requires each student to complete level C or BLS CPR. First Aid Training is not required by the programs.

Many CPR cards identify validity for two or three years.  Please provide the OT Program with a digital copy of your card showing the expiration date. If your certification requires you to renew to remain valid you must do so. Your CPR certification must remain valid throughout the duration of the program and it is the responsibility of the student to keep their certification valid.

For individuals completing CPR outside of Canada, you must ensure equivalency to CPR-Level C, and mail proof with a copy of your card

ACTION ITEMS

  1. Complete a CPR or re-certification session. The OT program does not provide on-site CPR sessions.
  2. Take a photocopy, scan or photo of your CPR certification.
  3. Upload the document to the Avenue to Learn assignment box (instructions below).
  4. For individuals completing CPR outside of Canada mail proof of equivalent standards.

COVID -19 MODIFIED POLICY

Due to the COVID-19 pandemic CPR Certification courses are not currently being offered. Incoming students are expected to obtain CPR Certification as soon as courses are being offered.

REGISTRATION & COURSE SELECTION

BACKGROUND
All new and returning graduate students are required to register annually on Mosaic.

ACTION ITEMS

  1. You should have already received communication from the School of Graduate Studies regarding your MacID, your barcode, and how to activate your MAC ID.  If you have not yet activated your MacID, please do so by clicking on this link.
  2. Login to Mosaic to enrol in courses.
  3. Follow the detailed course enrolment instructions

  4. Students must register for all first year level 700 courses within the OT Program:
    • Fall 2020 Term: 761, 771, 781, 791
    • Winter 2021 Term: 762, 772, 782, 792
    • Spring/Summer 2021 Term: 763, 773, 783
  5. Students must also register for SGS 101 and SGS 201 for the Fall 2020 Term.
    (Please note: First year students must register for SGS 101 and SGS 201. If you have previously taken these modules at McMaster University, you are not required to repeat these courses).
  6. Begin payment process - you will be prompted in MOSAIC regarding the deadline for graduate tuition fees.

STUDENT ID CARD – PHOTO COLLECTION FORM

BACKGROUND
Newly admitted students will upload a passport type photo (head shot) in Mosaic Student Centre>Personal Information>Demographics. Once you have completed your enrolment, your ID card will be produced.

If you already have a McMaster student ID card you will be required to continue to use your old ID card as a graduate student.

PHOTO REQUIREMENTS:

To be considered acceptable, your uploaded photo must:

  • be recent and similar to a passport photo (head shot only, looking straight into the camera)
  • have a neutral or smiling expression
  • be in colour, in focus and of good quality (scanned photos of your driver’s license, passport or health card are not acceptable)
  • be on an uncluttered background and of you alone (no other people or animals permitted in photo)
  • not contain props (i.e. hats, sunglasses or graduation caps)

Maximum file size of your photo must be 3MB or less to be successfully uploaded.
Uploaded photos not meeting the above criteria, will be deleted resulting in delays in receiving your student card.

Troubleshooting Tips:
Having issues uploading?

  • Check your file size. If your jpg image is too big, you will get a generic error message.  The file should be less than 3MB in size.   Try picking a different picture with a smaller file size or  resize your image in Paint or Photoshop.

Photo Upload button not active?

  • Call the Registrar’s Office (905-525-4600) during business hours for assistance.

HOW TO OBTAIN YOUR ID CARD:

  • We are currently unsure of if/how cards will be distributed due to COVID-19. More information will follow.
