SLP PROGRAM - YEAR 1

In the tabs below, you will find detailed information, specific to your first year in the program.  Please pay special attention to the Year 1 requirements which all have specific deadlines prior to starting in September.  All  requirements must be received by McMaster University by the stated deadline.  If requirements are not received by the deadline, program admission may be denied and the deposit forfeited.


Year 1 Requirements

Each item below has specific instructions on completing and submitting the requirements in addition to a contact person should you have any questions.

Some placement sites have additional requirements (e.g., site-specific health and safety training, more frequent police checks, CPR training, additional paperwork, etc.). It is the student’s responsibility to inquire about additional requirements and then ensure they have met the additional requirements prior to the start of placement. It is therefore strongly recommended that all students be certified with CPR level C or HCP by October 31.

DEPOSIT FEE

Deadline: Beginning June 1, 2017

Upon accepting your offer of admission to the program, learners are required to pay a $500 Deposit Fee to secure your place in the program. This deposit is non-refundable and the total amount will be credited towards your first semester’s tuition.  Deposit payments to be completed in Mosaic.  Once you have logged into MOSAIC go to 'Student Centre' and then the 'Accounts' page.

FINAL TRANSCRIPT SUBMISSION

Deadline: July 31, 2017

In addition to transcripts submitted to ORPAS, McMaster University requires a final official transcript mailed directly from your post-secondary institution to confirm that you have cleared academic conditions (e.g., degree conferred by June 30 and/or maintaining good academic standing for in progress courses).  Transcripts must be received in a sealed envelope directly from your post-secondary institution.  Transcripts should be mailed to:

VULNERABLE SECTOR SCREENING

Deadline: July 31, 2017

The Faculty of Health Sciences (FHS) requires incoming students to provide a Police Records Check/Vulnerable Sector Screen (VSS).  Turnaround time at many Police Services can be up to 12 weeks so don’t delay!  You must obtain this from your local police service and you will need identification showing an address in the same municipality as the police service.  Police check records are not returned, so request 2 copies so that you can retain one for your own records.  For full instructions and forms, go to:

HEALTH SCREEN RECORD

Deadline: July 31, 2017

The Faculty of Health Sciences (FHS) requires incoming students to provide a Health Screening Record in order to ensure your safety and that of your patients.  Some vaccination procedures take several weeks to be completed, so get started right away!

You must submit on time even if you don’t have all the supporting documents.

The Health Screening Record must be completed by one of the following professionals:  physician, nurse practitioner, registered nurse, physician assistant.

You should retain the original copy of your Health Screening Record.  The preferred method to submit your Health Screening Record is via secure online upload (available via the first link above).  Alternatively, you may submit via confidential fax to 905-528-4348 or else deliver a copy either in person or via mail to:

ANATOMY CARD & HOSPITAL BADGE

Deadline: July 31, 2017

Each student requires an anatomy card/hospital ID badge for numerous purposes throughout the program.  ID badge photos will be taken during Orientation Week and you will receive your card in September.

Prepare a cheque for $10.00 payable to “McMaster University” and include your full name and student number on the back of the cheque.  Mail to:

REGISTRATION & COURSE SELECTION

Deadline: August 29, 2017

Please note: failure to complete the academic portion of registration by this date invokes a late fee of $50.00

All new and returning graduate students are required to register annually on Mosaic.

  1. You should have already received communication from the School of Graduate Studies regarding your MacID, your barcode, and how to activate your MAC ID. If you have not yet activated your MacID, please do so by clicking on this link.
  2. Login to Mosaic to enrol in courses.
  3. Follow the detailed course enrolment instructions

  4. Students must register for the following courses:
    - All 700 level courses within the SLP program
    -SGS 101
    -SGS 201
    (Please note: First year students must register for SGS 101 and SGS 201. If you have previously taken these modules at McMaster University, you are not required to repeat these courses).
  5. Begin payment process - you will be prompted in MOSAIC regarding the deadline for graduate tuition fees.

STUDENT ID CARD – PHOTO COLLECTION FORM

Deadline: Prior to entry into program

Newly admitted students will upload a passport type photo (head shot) in Mosaic Student Centre>Personal Information>Demographics. Once you have completed your enrollment, your ID card will be produced.

If you already have a McMaster student ID card you will be required to continue to use your old ID card as a graduate student.

Photo Requirements:

To be considered acceptable, your uploaded photo must:

  • be recent and similar to a passport photo (head shot only, looking straight into the camera)
  • have a neutral or smiling expression
  • be in colour, in focus and of good quality (scanned photos of your driver’s license, passport or health card are not acceptable)
  • be on an uncluttered background and of you alone (no other people or animals permitted in photo)
  • not contain props (i.e. hats, sunglasses or graduation caps)

Maximum file size of your photo must be 3MB or less to be successfully uploaded.
Uploaded photos not meeting the above criteria, will be deleted resulting in delays in receiving your student card.

Troubleshooting Tips:
Having issues uploading?

  • Check your file size. If your jpg image is too big, you will get a generic error message. The file should be less than 3MB in size. Try picking a different picture with a smaller file size or resize your image in Paint or Photoshop.

Photo Upload button not active?

  • Call the Registrar’s Office (905.525.4600) during business hours for assistance.

How to Obtain Your ID Card:

  • Ontario students – ID cards will be mailed to your home address.
  • Students from outside of Ontario – visit the Registrar’s office for in-person pick-up.
  • International Students – visit the International Student Services (ISS) Office for in-person pick-up.

REGISTRATION WITH STUDENT ACCESSIBILITY SERVICES

Deadline: ASAP

If you have a chronic condition, disability or had accommodations during your undergraduate degree, we strongly recommend that you register with Student Accessibility Services (SAS).  Book an appointment as soon as possible as accommodation contracts may take time to finalize.

Students who require support personnel to participate in the program are required to contact SAS and SAS will facilitate the completion of all relevant Program Requirement Action Items for needed support personnel.

If you live out of town, please ask SAS for a teleconference to discuss your needs.
