SLP PROGRAM - YEAR 2

In the tabs below, you will find detailed information, specific to your first year in the program.  Please pay special attention to the Year 1 requirements which all have specific deadlines prior to starting in September.  All  requirements must be received by McMaster University by the stated deadline.  If requirements are not received by the deadline, program admission may be denied and the deposit forfeited.


Year 2 Requirements

Each item below has specific instructions on completing and submitting the requirements in addition to a contact person should you have any questions.

VULNERABLE SECTOR CHECK

Note: You must have a valid VSC at all times in the program.  DO NOT ALLOW YOUR VSC TO LAPSE.

To remain in good standing, you must provide an original copy of your Police Records Vulnerable Sector Check (VSC) to the FHS Professionalism Office on an annual basis and prior to your current check expiring. If you fail to do so you may be considered in breach of the Police Records Check Policy. It will take up to 5 business days for your VSC to be processed by the Professionalism Office, and for the program to receive verification, so plan accordingly. Note that the original copy you submit will not be returned to you unless you include a self-addressed stamped envelope. Students are responsible for making their own copies.  If you require a copy from the professionalism office, a fee of $30 will be charged (see program handbook).

*Note: Some placement sites require more frequent Vulnerable Sector Checks, such as every 6 months. It is your responsibility to ensure you meet the individual requirements of any sites where you are placed.

HEALTH SCREEN RECORD

Deadline: July 31, 2019

These are mandatory requirements for continued participation in clinical activities.

  • Students granted full clearance in Year One are required to submit annual tuberculosis (TB) screening.
  • Students granted provisional clearance are required to complete all outstanding requirements, in addition to annual TB screening.  This includes adult pertussis (Tdap), all outstanding vaccinations for tetanus/diphtheria/polio, and Hepatitis B vaccinations/serology.
  • Students returning from a leave of absence are required to complete all outstanding requirements and any updated requirements, in addition to annual TB screening.

CPR TRAINING (BLS LEVEL)

The Faculty of Health Sciences supports a 2-year re-certification period for CPR Training (BLS). Students may therefore need to re-certify to ensure they are covered until the end of the program.

Year 2 Enrollment

Opens: July 23, 2019
Deadline: August 27, 2019

Please note: failure to enroll in all required courses by the deadline will result a late fee of $50.00, applied to your student account.

All new and returning graduate students are required to register annually on Mosaic.
